Send a referral
Call the front desk, fax the referral to (502) 555-0105 or use the secure form. We confirm that we have it and call the patient the same working day.

The patient's name and phone, your contact details, the area of concern, any precautions and, after surgery, your written protocol.
How we follow a protocol
As written, including precautions and limits on motion. If an exam does not fit the protocol, we call your office before changing anything.
What you get back
A written note after the first visit and whenever the plan of care changes, sent the way your office prefers.
Who treats your patient
Five physical therapists, each with a manual-therapy fellowship. Tell us the body part and the procedure, and the front desk books the clinician who treats it most.
